领取须知：

1. 已领取奖项的选手请勿在此申领；
2. 通过地区承办的选手，请优先咨询承办单位，奖项将会打包给承办单位，并由承办单位统一分发；
3. 请如实填写完整的参赛编号、手机号码、收件地址等信息，并再三确认所填信息准确。如填写错误信息，导致无法邮寄、丢失及所产生的一切费用，主办机构全无责任，因此再次补领的一切费用将由申请者承担；
4. 已提交过申请奖项的，请勿重复提交。奖项将会按照申领先后次序顺序发放，请各位耐心等待收货；
5. 本次补申领日期截至于2025年9月30日23:59:59。

To reduce waiting time, please follow the collection instructions below:

Award Collection Notice:

• Contestants who have already collected their awards should not apply here.
• Contestants who participated through a regional organizer should first consult their respective organizer. The awards will be sent in a package to the organizer for unified distribution.
• Please accurately fill in your complete contestant number, mobile phone number, mailing address, and other information, and double-check to ensure its accuracy. If incorrect information is provided, leading to delivery failure, loss of items, or any associated costs, the organizing committee will not be held responsible. Therefore, any expenses incurred for reissuing the award must be borne by the applicant.
• If you have already submitted an application for your award, please do not submit it again. Awards will be dispatched in the order that applications are received. We kindly ask for your patience in awaiting delivery.
• The deadline for this supplementary award application is September 30, 2025, at 23:59:59.

1. 此申请表仅适用于参加第十二届香港国际音乐节2025总决赛的第一名 (奖金CNY 2000元) 、第二名 (奖金CNY 1000元) 、第三名 (奖金CNY 500元) 获奖选手填写申报，如有奖项名次并列，则奖金平均分配。
2. 本次申领日期截止于2025年9月30日17:00:00前，若在此截止日期之前未提交申请，则将视为主动放弃奖金申领。
3. 请勿重复提交申请，组委会根据提交先后顺序陆续发放奖金，敬请耐心等待。

Scholarship Application Notice    
To reduce waiting time, please adhere to the following requirements:

1. This application form is only applicable to the top three winners participating in the 12th Hong Kong International Music Festival 2025 Finals: First place (Prize money CNY 2,000), second place (Prize money CNY 1,000), third place (Prize money CNY 500). If there is a tie for rankings, the prize money will be distributed equally among the tied winners of the first, second and third place.
2. This application must be submitted by 5:00 p.m. on September 30, 2025. If the application is not submitted within this period, the prize will be deemed to have been automatically given up.
3. Please do not submit repeated applications. The organizing committee will distribute prizes in order of submission time, your patience is appreciated.
